Emergency Medical Release

As a parent or guardian, I authorize the treatment of all listed children on this registration form by a qualified and licensed medical doctor in the event of a medical emergency which, in the opinion of the attending physician, may endanger his/her life, cause disfigurement, physical impairment, or cause undue discomfort if delayed. This authority is granted only after a reasonable effort has been made to reach me. I will not hold Waypoint Community Church or any of the adult leaders responsible for any injury or inconvenience that might arise during any AWANA or related youth ministry activity. I hereby grant permission to the leaders to request emergency medical treatment for all listed children. This release is granted for the dates beginning September 01, 2026 and ending August 31 2027. It is completed and signed of my own free will.

Media/Photo Release

I, the parent or guardian of the child(ren) listed on this registration form, give Awana at Waypoint permission to take photos and videos of my child(ren) and to use them in our end-of-year slideshow, and also to potentially use in flyers and the promotion of Awana at Waypoint program and events.

Clubber Code of Conduct

Our Purpose

Our goal is to provide a partnership with parents in discipling their children in a safe, fun, and Christ-centered environment where children can grow in their relationship with Jesus Christ. We ask every clubber and family to support these expectations so that everyone can enjoy their AWANA experience!

Clubber Expectations
● Treat leaders, helpers, and other clubbers with kindness and respect.
● Listen to and follow instructions from AWANA leaders.
● Use respectful words and actions.
● Keep my hands, feet, and objects to myself unless participating in an approved game or activity.
● Take care of church property, AWANA materials, and the belongings of others.
● Participate with a positive attitude and encourage others.
● Be honest in my words and actions.

Behaviors That Are Not Acceptable:
● Bullying, teasing, intimidation, or harassment.
● Fighting or intentionally hurting others.
● Disrespectful language, profanity, or inappropriate jokes.
● Defiance or repeated refusal to follow directions.
● Intentional damage to property.
● Possession of any dangerous or prohibited items.
● Any behavior that disrupts the safety or learning of the club.

Our leaders will seek to correct behavior with grace, patience, and consistency. Depending on the situation, consequences may include:

● A verbal reminder or warning.
● A corrective conversation with a leader.
● Temporary removal from an activity.
● Parent notification.

If serious or repeated behavior continues, the clubber may be asked to leave for the evening or, in rare cases, may be suspended from attending one or several future club nights.

Our desire and focus is always restoration, learning, and helping each child grow in Christ-like behavior! If there is a way to resolve an issue peacefully between all parties, our expectation for our leadership is to do so, and we strive to take into consideration any special social needs a clubber may have.

Parents and guardians agree to:
● Review these expectations with their child.
● Support AWANA leaders in maintaining a safe and respectful environment.
● Be available to discuss concerns if contacted by club leadership.
● Pick up their child promptly if requested due to illness, injury, or significant behavioral concerns.
● Communicate to us if they will be late to pick-up their child.

Mobile Device Policy
● Clubbers are expected to enjoy their time at AWANA without the distraction of mobile devices. If it is not possible for your clubber to leave the screen at home, then they may check it in with the appropriate club secretary at the beginning of the evening (exceptions being devices used for legitimate health reasons, eg: continuous glucose monitors and the like).
● The club secretary will hold the device for the clubber to use if a need arises, and the clubber can pick up the device at the end of the evening. The clubber may check the device for communication between each club activity as needed.
Please select all that apply.

Volunteer at Awana

We are always on the lookout for more volunteers to join our AWANA family on Wednesday nights. Whether by just coming and listening to verses at handbook time, or by serving as one of our amazing leaders, there are many ways to participate and help! If you or someone you know is interested in volunteering at AWANA this year, please fill out the information below and we will be in contact with you or them. Leaders receive a discounted registration cost for their clubbers, and are eligible to have their preschool-aged children attend our Cubbies club, which is provided exclusively for the children of volunteers. Thank you so much for your consideration, we are excited to see what God has in store for us this year!

One-Time Donation

If you would like to make an extra one-time donation to help further support AWANA at Waypoint, please indicate so in the box below by entering the amount that you would like to give as support. We don't desire anyone to give outside of their means. However, we know that there are some people who would like to give more in support of this ministry that has such a long and rich history at this building, and so we have made that available to you.
